Doane Family Enterprises offers one dental plan, a DPPO Plan, through United Concordia. The United Concordia PPO Dental Plan allows you to see any dentist you choose. However, you will save more money when you use In-Network PPO dentists. In addition, in-network dentists will file claims for you. If you use an out-of-net-work dentist, the benefit is subject to reasonable and customary reimbursement.

Searching for an in-network provider – Through www.unitedconcordia.com and the United Concordia Dental Mobile app (download on Apple or Android), you can find an in-network dentist.

To search for a provider before you are an enrolled, registered member:

Go to www.unitedconcordia.com/findadentist
You will first enter your location by either City and State or Zip code
Select the network Concordia Advantage
Once the information is entered, a list of dentists in your area will populate. You can either search for providers by name or practice, or sort and refine the search by type of dentist or provider you are looking for to refine the list of providers in your area

You may register www.unitedconcordia.com/MDB. Then click on Create an Account. Select Member and use the information on your member ID card along with your Date of Birth to complete your registration. You can also use the Social Security Number of the policy holder in place of the ID Card. Each dependent with United Concordia dental benefits must create their own My Dental Benefits Account.
