Medical: Visit https://www.ibx.com/get-care/find-doctors-and-health-care-providers or the IBX mobile app (download on Apple or Android) to find an in-network doctor.
Dental: Visit www.unitedconcordia.com or the United Concordia Dental mobile app (download on Apple or Android) to find an in-network dentist.
To search for a provider before you are an enrolled, registered member:
Go to www.unitedconcordia.com/findadentist
You will first enter your location by either City and State or Zip code
Select the network Concordia Advantage
Once the information is entered, a list of dentists in your area will populate. You can either search for providers by name or practice, or sort and refine the search by type of dentist or provider you are looking for to refine the list of providers in your area
Vision: Visit www.vsp.com or the VSP Vision Care mobile app (download on Apple or Android) to find in-network vision providers.
To search for a provider before you are an enrolled, registered member, visit https://online.visionbenefits.com/Search/default.aspx
Medical: Login to www.ibx.com. Once logged in, you'll have access to print or download an ID card. You may also call Independence Blue Cross at 1-800-275-2583 to request a new ID card.
Dental: Login to www.unitedconcordia.com. Once logged in, you'll have access to print or download an ID card. You may also call United Concordia at 1-800-332-0366 to request a new ID card.
Vision: VBA has simplified the process. You do not need an ID card to make an appointment or visit your in-network provider.
When making your appointment, tell your in-network provider you have coverage through VBA.
You will need to provide your social security number to your VBA in-network provider.
If you would like a VBA Member ID Card, you can print one from the VBA Member Portal.
You may also call them at 1-800-432-4966 to request an ID card.
Visit www.ibx.com, click on Login/Register, and follow the prompts using your ID card or personal details (DOB, SSN, Zip) to sign up.
As a member, you can estimate costs, view claims, locate network providers, get prescriptions delivered, and access 24/7 online medical consultations.
Use the IBX mobile app for easy health plan management on your smartphone.
To find an in-network provider or pharmacy, use the "Find a Doctor" feature on the website or app, select "Personal Choice PPO" network, update your location, and search by provider name or specialty, even before registration.
You are required to update your Independence Blue Cross Coordination of Benefits information every year so that they can properly process your claims.
Coordination of Benefits information confirms whether you and/or your enrolled dependents have insurance coverage other than your employer's and if so, what that other insurance is, so that they can determine proper payment responsibilities for claims. If your Coordination of Benefits is updated yearly or as requested, your claims will be processed.
